Imatinib Therapy in Chronic Myelogenous Leukemia
نویسنده
چکیده
Chronic myelogenous leukemia (CML) is a clonal proliferative disease originating from a pluripotential stem cell in which a 9:22 translocation, Philadelphia (Ph) chromosome, results in the production of BCR-ABL fusion protein that has constitutive tyrosine kinase activity and deregulates signal transduction pathways, thus causing leukemia. The natural course of CML is usually triphasic (chronic, accelerated and blast-crisis phases). The blast-crisis phase is refractory to standard chemotherapy. The introduction of imatinib, a selective inhibitor of the BCR-ABL tyrosine kinase, has caused rapid change in the treatment protocols for patients with CML. The great efficacy of imatinib in achieving a cytogenetic response has been demonstrated in many trials, but long-term clinical data regarding imatinib are not yet available.

Pregnancy Outcome of Two Patients with Chronic Myelogenous Leukemia Treated with Imatinib
Although chronic myelogenous leukemia in pregnancy is rare, its management and treatment is more difficult and complicated.Treatment of patients with chronic myelogenous leukemia includes bone marrow transplantation, however in less than 30% of patients the donor’s organ would be accepted. To this end, cytotoxic therapy is considered as an alternative therapeutic option. This option provides sa...
متن کاملEvaluation of Thyroid Dysfunction during Imatinib Therapy in Chronic Myeloid Leukemia
Background: Imatinib mesylate is the first generation of Tyrosine kinase inhibitors (TKI) and highly effective in the treatment of chronic myeloid leukemia (CML). We aimed to evaluate thyroid function at baseline and at 1, 3, 6 and 12 months after initiation of Imatinib mesylate therapy in 20 newly diagnosed BCR-ABL positive CML patients. Methods: This study was done during 2013-2014, 20 new c...
متن کاملOutcome of patients with Philadelphia chromosome-positive chronic myelogenous leukemia post-imatinib mesylate failure.
BACKGROUND The prognosis of patients with chronic myelogenous leukemia (CML) after failure of imatinib mesylate therapy is not well documented. METHODS The outcome of 420 patients with CML post-imatinib failure (resistance-recurrence in 374; toxicities in 46) were reviewed in relation to survival, overall, and by different therapies. RESULTS The estimated 3-year survival rates were 72% in 8...
متن کاملBCR-ABL transcript variations in chronic phase chronic myelogenous leukemia patients on imatinib first-line: Possible role of the autologous immune system.
Many chronic myelogenous leukemia (CML) patients in chronic phase who respond well to imatinib therapy show fluctuations in their leukemic loads in the long-term. We developed a mathematical model of CML that incorporates the intervention of an autologous immune response. Our results suggest that the patient's immune system plays a crucial role in imatinib therapy in maintaining disease control...
متن کاملSurvival benefit with imatinib mesylate versus interferon-alpha-based regimens in newly diagnosed chronic-phase chronic myelogenous leukemia.
A survival benefit for imatinib mesylate versus interferon-alpha therapy could not be demonstrated in the randomized study in newly diagnosed Philadelphia chromosome (Ph)-positive chronic-phase chronic myelogenous leukemia (CML) due to the high rate of crossover (90%) from interferon-alpha to imatinib mesylate within a year of study entry. We compared survival in 279 patients with newly diagnos...
متن کاملA comparison of donor lymphocyte infusions or imatinib mesylate for patients with chronic myelogenous leukemia who have relapsed after allogeneic stem cell transplantation.
Imatinib mesylate is highly effective in relapsed chronic myelogenous leukemia (CML) after allogeneic hematopoetic stem cell transplantation (HSCT). However, it is unknown whether imatinib produces durable molecular remissions. The outcome of CML patients transplanted at our center who had received only imatinib for relapse after HSCT was compared with that of patients treated with donor lympho...
